How it works

Case Conferencing is the recurring, multi-agency meeting that is the operational engine of the whole system. Providers come together around a shared list and take a real, owned next step for every person — not a status update, an action with a name and a date attached.

The key elements

  • 01Population-segmented meetings — veterans, chronic, families, and general/non-chronic, in person at least monthly, so the right providers are in the room.
  • 02The Learning Loop, per client — name & time on the list, action since last meeting, the largest barrier, a next step with owner and due date, and a target move-in date.
  • 03Target move-in dates as the discipline — a missed date is treated as system learning, not a staff judgment.
  • 04Trained facilitation — HOPE-team facilitators, a rotating scribe and timekeeper, and a clean-the-list prep routine three days out.
  • 05A feedback loop to leadership — recurring barriers roll up to the Champions Table and Improvement Teams for path-clearing.
